Audio Running But Not Video on Pre-Roll Ads

We have pre-roll video ads running:

But only the audio is showing up and not the video.

We are using VAST/VPAID tags from adaptv

I see a preroll ad here (football ad), with both audio and video.

Then, another ad runs, from ESPN.

Then, another ad runs, some sort of Doctor ad. All of the ads run fine though, with video and audio.
