For some reason, the labels on the HD toggle button are not showing up. I have them in the RSS file.
Has the formatting changed? What should I be putting in there for the HD label to appear?
Your first playlist item only has a single source:

All other videos in the playlist are displaying the HD toggle button.
Yes. The Ironman clip is only one video. However, the others have two different versions each.
I believe there is a misunderstanding on the HD button. When I hover my mouse over it, I thought I would see the other available options - similar to the Closed Captioning where it will show you the different languages.

Please note: I am using a different skin than the one in the example above. I was just curious why that option is no longer available.
Correction: That little pop up window does not work on the default screen either.
If only two sources are used, the player will set this as a toggle, rather than a selection menu.
So you're saying I need to have more options?
